After downloading one of the forms below, email, fax, or mail them to CLF at the contact information on the form (or on the "Contact Us" link on the left bar).

NOTE:  CLF is affiliated with The National Christian Foundation (NCF), and some of the forms below reference NCF (as well as Fidelity).

Wire Notification Form

• Wire Notification Form – to notify CLF that you are wiring money to your Fund at CLF

Giving Securities

• Letter of Authorization for Stock Transfers – to transfer stock on deposit in your brokerage account. If your shares are held at Computershare, please contact Computershare and request your shares in certificate form. Computershare will send you a certificate, and then use the Certificate Release Request and the Irrevocable Stock Power Form below to transfer your stock certificate to CLF.
   
• Letter of Authorization for Mutual Fund Transfers – to transfer mutual funds to CLF.  Select the institution you're transferring from:
  • Fidelity - this form may also be used to transfer mutual funds if they are held at a firm other than the ones listed. Check with your broker to make sure the mutual funds are transferable. If they are not transferable, contact us at 813-567-1499.

• Certificate Release Request – to transfer stock certificate(s) in your physical possession
   
• Irrevocable Stock Power Form (Fidelity Form) – to allow CLF/NCF to facilitate the processing of your certificate(s) in the absence of your endorsement on the certificate(s)

Testamentary giving forms

• Testamentary Gift Information - explains the process for naming CLF as the beneficiary in your will/estate 
   
• Giving Fund Succession Plan Letter of Advisement - allows you to recommend to CLF where your Giving balance is distributed upon your death 
    

Grant recommendation forms

• Domestic Grant Recommendation Form – for grants to U.S.-based charities (though recommending grants is free and fast on our Donor Website)
